Welcome to this demo survey!

As a new feature we have enabled the "Bubble style progress bar" which looks a little more fancy. But you can always switch back to the default layout if needed.

There are 28 questions in this survey.
This survey is anonymous.

The record of your survey responses does not contain any identifying information about you, unless a specific survey question explicitly asked for it.

If you used an identifying access code to access this survey, please rest assured that this code will not be stored together with your responses. It is managed in a separate database and will only be updated to indicate whether you did (or did not) complete this survey. There is no way of matching identification access codes with survey responses.

Mask Questions

This is a mixture of special question types provided by Limesurvey.

The "date" question type uses a calendar to let users pick a date.
Open date/time selector
There are additional question settings to adjust the calendar. Here we set the min. year to "2000".
A fancy drag and drop feature to rank items.
Using the equation question type you can do different calculations and store the results at the database.

To give you an example, we count the number of items ranked at the previous question: You have ranked 0 items so far.

Limesurvey even offers a file upload question type. Feel free to upload up to 2 files.
  Title Comment File name
max file size: 200KB
allowed file types: png, gif, doc, odt
max upload: 2 files
Array Questions
Here come some different array questions:
A simple array question with demo texts.
vero wisi veniam hendrerit rebum suscipit molestie No answer
Consetetur sadipscing elitr eirmod tempor invidunt
Answer width = 30
The "semantic differential" is a variation of the array question type. You can define your own answer options on top and define the text for the scale on the left/right.
- - - - / + + + + No answer
bad good
unimportant important
unlikely likely
At the "Array by column" question type the axes are reversed. That means you can pick only one item per column.
vero wisi veniam hendrerit rebum suscipit molestia
Consetetur sadipscing elitr eirmod tempor invidunt

If you want to ask for different text information for a set of items this question type is the best choice.

This is a variation of the "Array texts" question type. Here the input is limited to numbers and Limesurvey politely calculates the row and column totals for us.

We can use the same layout as above and use a question of type "Array numbers" to offer a predefined list of numbers ranging from X to Y:
Please mark the possessions of the following people:
House Car Dog iPad
My parents
My mother's parents
My dad's parents
Using the "Array numbers" question type is the best way to present a matrix of checkboxes.
To do so you have to set "Checkbox layout" = "Yes" at the advanced question settings.
The "Array dual scale" question can be used if you need two different scales e.g. for rating certain features.

Example: "Please rate 'quality' and 'price' of the following cars:"
Quality Price
very bad bad good very good
No answer
cheap moderate expensive No answer
Ford Focus Quality
Price
VW Passat Quality
Price
Audi A6 Quality
Price
Toyota Corolla Quality
Price
For larger answer scales you can use a dropdown for each scale.
Multiple choice questions
Multiple choice questions can either be a simple list of checkboxes or a set of multiple numeric inputs. The later can use a slider layout as well.
A simple multiple choice question with demo texts. Watch the question below when checking different items.
The items of this list are split up into two columns.
The last item is set as exclusive option.
The "Array filter" feature is applied to this question in order to only list items which were selected at the previous question.
The "Multiple numerical input" question type allows to assign a number to each item.
This is a variation of the "Multiple numerical input" question type using sliders. At this example the sum must equal 100!

Single choice questions
For the "List radio" question type you can define all answer items yourself.
Single choice questions with lots of answer options better use this drop-down layout.
Using the last "other" option you can enter your own answer.
This "5 point choice" can be used for rating.
A simple "Yes/No" question with predefined answer options.

If you want to ask for the participant's gender, use the "Gender" question type.
Text Questions
Depending on the amount of text you want your users to enter you can select one of the following question types:
If you want to ask for the participant's age, the "Numerical input" question type is a good choice.
You can use prefix and suffix like this:
I am
years old.
The "Short free text" question type looks like this:
For longer texts you can use the "Long free text" question type...
...and if your users should input lots of text, use the "Huge free text" question.
When asking e.g. for address data, a set of several input fields is needed:
If you need a set of several larger text input fields, the "Short free text" question can use a textarea.
To make the short text question use a larger textarea, use the 'display rows" (here = 4) advanced question setting.
Additionally, we have set the 'Input box width' to 50.
The "Short free text" question type can be used to store map data (latitude and longitude values).
